You will fill out the one page Claim Form. (If you have trouble with this, you will contact the Small Claims Advisor who is in the courthouse every morning in Room 107 and can be visited on a first come first served basis from 8-11 a.m. He/she is also available by telephone every afternoon from noon to 4 p.m. at (707) 524-7349, and will help answer your questions.) You must type the form, as handwritten forms are not accepted.
